
What is an ACL Injury?
Anterior cruciate ligament injury is a common knee injury. This ligament is torn when knee suddenly gets twisting injury while fall from two wheeler or fall during football etc. The knee gets swollen and there is sound heard from knee and walking is painful/ ACL tear results in knee instability. Patient feels lack of confidence on knee, the knee suddenly pivots/twists and running and jumping is practically impossible. How do I evaluate?
ACL surgery is done under local/regional anesthesia and usually takes about 30-40 minutes. Patient gets admitted on same day of surgery. After anesthesia, I do diagnostic arthroscopy. This involves a small puncture in front of knee and inserting telescope in knee to look at all structures of knee. The image of screen is seen on TV screen with magnification. I make another puncture for instrument insertion into knee and to hold and feel meniscus, cartilage and ligaments of knee. Once I have evaluated extent of damage.
We repair structures one by one. Meniscus tear is repaired with meniscus repair devices. Meniscus is cushion between knee and tears usually with ACL. It is an important structure and we need to repair it to get best possible results. Cartilage is repaired using different cartilage stimulation techniques and devices.
How do I take care of an ACL injury?
The way ACL surgery is done depends upon on type of tear – Partial or Complete and also place of tear – Mid-substance or Pulled off from bone. If I find the ligament healthy and is pulled off the bone, I do ACL repair that means stitch the ligament back to place from where it is torn. This restores normal shape and strength of ligament and gives good outcome.
If ligament is badly torn especially from middle, then the repair is usually not advisable. In such cases, we do ACL reconstruction. This involves taking tendon from just below knee. I give small incision 2 cm below knee and take out one hamstring tendon. This tendon is used to make ligament. The tendon is strong enough once we fold it four times and is fixed in bone under guidance by telescope.
As the tissue is taken from patient himself, the acceptability is 100% unlike artificial graft or material which body may reject. The surgery is precise and experience helps me to put ligament in exact position and tension. As i treat national athletes, I fully understand that all associated structures needs to be repaired as well to get best results.

When can you walk after the ACL Reconstruction?
All my patients are able to start walking next day putting weight on leg. The operated leg is supported with long knee belt which is worn while walking to avoid sudden jerk or twist. Hospital stay is usually of one day. Stitches are about 5-6 and are removed at 10th day. Knee bending is started next day of surgery. It is 30 degree bending in Ist week, increase to 60 degree next week, 90 degree by 3rd week and full bending by 4th week. Belt is also removed completely by 4 weeks.
