How common is a frozen shoulder?

Frozen shoulder is seen in 10-15% cases of painful shoulder. It is common in diabetics and those with thyroid problems. Shoulder specialist by doing certain clinical tests can differentiate between frozen shoulder and other causes of painful shoulder.

How do I evaluate?

Patient with primary frozen shoulder have resisted shoulder movements in all directions, the pain is worse in night and on moving shoulder joint, small injury or jerk may result in frozen shoulder. Xray are usually normal and MRI might show inflammation and small tears in tendons which may not be significant.

How I treat it

Most frozen shoulder patient get with rest, physiotherapy, modification of daily activities, gentle range of motion exercises and analgesia. Injection might be helpful in some patients when inflammation is not getting well with physiotherapy and they are extremely painful. Single dose of injection is usually good but in some cases may need additional injection after few days.

What is the role of arthroscopy?

Patient with very stiff joint who are unable to cope up with their daily activities may need shoulder arthroscopy capsular release. This is a procedure in which under anaesthesia, a telescope is put in shoulder and with the help of special instruments/devices, the tissue capsule is released and patient gets back his shoulder movements instantly.
